We accept most traditional insurance plans, but we recommend contacting our office at (850) 729-3300 to verify your specific coverage. While we accept most traditional plans, we kindly ask that you review your own insurance benefits prior to your visit. Understanding your specific co-pay and deductible amounts beforehand helps avoid any surprises at check-in. We recommend calling the member service number on the back of your insurance card before your appointment to ask these key questions:
Is this provider in-network? Confirm that our office is part of your specific plan's network to ensure the lowest out-of-pocket cost.
What is my co-pay for the visit? Co-pays are fixed fees often due at time of service.
Have I met my annual deductible? A deductible is the amount you pay for covered services before your insurance begins to pay.
Do I have a co-insurance percentage? This is your share of cost for a service after your deductible is met.

For those without insurance, we are happy to offer a self-pay option. To show our appreciation for cash payments, we offer a small discount on our standard rates:
New Patient Visit: $315 (Credit Card/CareCredit) or $300 (Cash Discount)
Follow-up Appointments: $105 (Credit Card/CareCredit) or $100 (Cash Discount)
