Welcome to Emerald Coast Family Medicine! My name is DeAnn, and as the Office Manager, my goal is to make sure your experience with us is as worry-free as possible. One of the things that truly sets our practice apart is the direct relationship we build with our patients-at every single appointment, you will be meeting directly with Dr. Castaneda. What to Expect: Your First Visit
We want your first appointment to be as straightforward as possible. Here is a quick guide to help you prepare:
Arrive Early for Paperwork: Please plan to arrive 15 minutes before your scheduled time. You can download and print our New Patient Forms
The Check-in Process: Please bring your photo ID and current insurance card. We will verify your coverage and collect any applicable co-pays or deductibles at this time.
Meeting Dr. Castaneda: After you are checked in, one of our Medical Assistants will bring you back for a brief intake history review to prepare for your visit with Dr. Castaneda.
Next Steps: Before you leave, we will provide any necessary prescriptions and help you schedule any needed follow-up visits.
